Important information about heath services and requirements for new students. 

Welcome to the Health and Wellness Center

Before you arrive on campus

★ Submit your New York State required immunization records by July 1 for the Fall semester. Here's how:

  • Upload through the Student Health Portal (preferred)
  • Or send via Email: HWC@Utica.edu
  • Or Fax to 315-792-3700

What do I need to submit?

To meet New York State and university requirements, you must submit documentation showing proof of required immunizations.

You may submit ONE of the following:

Option 1: Official Immunization Records

Records from your healthcare provider, school, or state immunization registry. This must include:

  • Your full name
  • Date of each immunization
  • No provider signature is required
Option 2: Completed Health Form
  • Have your healthcare provider complete and sign the university health form
  • Submit the completed form to the Health and Wellness Center

Health Form

You only need to provide documentation for:

  • MMR (Measles, Mumps, Rubella) Requirement
    • Two doses are required
    • First dose after first birthday
    • Second dose at least 28 days after first dose
  • Meningococcal (Meningitis) Requirement
    • Proof of vaccination within the last 5 years, OR
    • Signed meningitis waiver/declination form

Please note:

  • You only need to provide documentation for the immunizations listed above. No additional vaccines are required for general student compliance.
  • A physical exam is NOT required for most students.
  • Some programs (such as Nursing, OT, and PT) may have additional health requirements. You will receive separate instructions if this applies to you.
  • You do not need a provider signature if submitting official records
  • All documents must be legible
  • Documents not in English must include a translation
  • Incomplete submissions may delay your clearance

All incoming on-campus students (6+ credits)

Not required for fully online students. Some programs have additional requirements:

  • Student-athletes
  • Nursing students
  • Occupational Therapy (OT) Students
  • Physical Therapy (PT) Students

You will receive separate instructions from your program on their additional requirements.

FAQs

Have questions? You’re not alone! Here are the most common questions students ask:

Q: Do I need a doctor’s signature?

No. If you are submitting official immunizations records, a provider signature is not required. If you are only submitting the health form without additional documentation, a doctor’s signature or office stamp is required.

Q: What counts as official immunization records?

Records from your healthcare provider, school, or state immunization registry that include your name and vaccine dates.

Q: Can I submit a screenshot or photo?

Yes, as long as all information is clear and legible.

Q: What if I only have one MMR shot?

You must receive the second dose and submit documentation. The second dose must be at least 28 days after the first. If you need a second dose, any local pharmacy should be able to provide it.

Q: What if my Meningitis immunization is more than 5 years old?

You can either get another immunization and provide that documentation or you can sign the meningitis waiver on the Health Form declining to receive another immunization and submit that instead.

Q: Do I need any vaccinations besides the MMR and Meningitis vaccinations?

No, New York State only requires proof of MMR (Measles, Mumps, Rubella) and Meningococcal vaccination or a signed waiver for college students. No other immunizations are required for general compliance. However, additional vaccinations may be required by your specific academic program. You will receive separate instructions from your program if this applies to you.

Q: Do I need to submit immunization documentation if I am not living on campus?

Yes, all students, regardless of where they live, need to submit immunization documentation if they are attending 6 or more credits of on-ground classes.

Q: What if I can’t find my records?

Contact your healthcare provider, high school, or state immunization registry to obtain a copy.

Q: What happens if I don’t submit my documents?

You will not be able to move on campus until your records have been received and cleared. You may also be unable to register and attend classes until the requirements are met.

Q: When can I access the Student Health Portal?

The portal opens after your deposit is paid and you receive your student ID number.

Q: How do I know if I’m compliant?

You will receive a confirmation email from the Health and Wellness Center to your Utica.edu email address after your documents are reviewed.

Q: Do I need to submit a physical exam form?

No, Most students do not need to submit a physical exam form. Some programs (such as Nursing, OT, PT and Athletics) will require additional documentation.

Q: What if I can’t log into my Student Health Portal?

You can either contact the Health and Wellness Center to have your portal access reset or you can submit your documentation via email or fax instead.

Q: Can I just hand in my documentation when I come for move-in?

No, you must be cleared in advance to move in. You will not be able to move onto campus or register for classes until cleared. Please submit documentation by July 1.
